1. Program description. If you consent to receive conversational and informational SMS from Support Plus Personal Care LLC ("SPPC"), you agree to receive conversational and informational text messages from us at the mobile number you provide. Message topics include replies to your questions; visit scheduling, confirmations, reminders, and changes; care-coordination logistics; requests for documents or signatures; and, for employees and applicants, shift, schedule, hiring, and other employment-related notices.
2. Consent is not required. Consent to receive text messages is not a condition of receiving services from SPPC or of employment with SPPC. You can receive the same information by phone or email.
3. Message frequency. Messaging frequency may vary. The number of messages you receive depends on your services, schedule, and interactions with us.
4. Cost. Message and data rates may apply. Your mobile carrier's standard charges apply to messages you send and receive.
5. Opting out. Reply STOP to opt out at any time. After you text STOP, we will send one message confirming that you have been unsubscribed, and you will receive no further text messages from us unless you opt in again. You may also revoke consent by any reasonable means, including replying END, CANCEL, UNSUBSCRIBE, QUIT, or REVOKE, calling 262-420-4008, or emailing [email protected]. We process opt-out requests promptly and in every case within 10 business days, and we treat an opt-out as applying to all text messages from SPPC. Opting out of text messages does not stop communications by phone, mail, or email that are necessary for your care or employment.

7. Carrier disclaimer. Carriers are not liable for delayed or undelivered messages. Supported carriers include major U.S. carriers; delivery is subject to carrier availability and effective transmission from your network operator.
8. Eligibility and accuracy. You must be at least 18 years old and the account holder or an authorized user of the mobile number you provide. You agree to notify us if your mobile number changes or is deactivated, so that we do not send messages to a number reassigned to someone else. For clients who are minors or who have a legal representative, text communications are with the parent, guardian, or authorized representative who opted in.
9. No emergencies by text. Text messages are not monitored around the clock and delivery is not guaranteed. Do not use text messages to report an emergency, a fall, an injury, a medication problem, or any urgent care need. In an emergency, call 911. For urgent non-emergency matters, call 262-420-4008.
10. Privacy and message security. Our Privacy Policy at supportpluspc.com/privacy-policy explains how we handle your information. No mobile opt-in or text message consent will be shared with third parties or affiliates. Standard text messages are not encrypted end to end and can be viewed by anyone with access to your phone or your carrier's records. Our messages contain only the minimum information needed and do not include detailed health information. By opting in, you acknowledge these risks. You may request a different method of communication at any time.
11. Changes. We may modify or cancel the program or these terms at any time. Material changes will be posted at supportpluspc.com/sms-terms or sent to you by text. Continued participation after a change means you accept the updated terms.
