Support Plus Personal Care LLC ("SPPC," "we," "us," or "our") is a Wisconsin-certified personal care agency serving clients in southeastern Wisconsin. This Privacy Policy explains how we collect, use, share, and protect personal information when you visit supportpluspc.com (the "Site"), contact us, apply for employment, receive services from us, or communicate with us by telephone, email, or text message (SMS/MMS).
By using the Site or communicating with us, you agree to this Privacy Policy. If you do not agree, please do not use the Site or opt in to our text-messaging program.
SPPC is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Information about your health, care, or the services you receive from SPPC ("protected health information" or "PHI") is governed by HIPAA (45 CFR Parts 160 and 164), applicable Wisconsin confidentiality law (including, where applicable, Wis. Stat. § 146.82), and our Notice of Privacy Practices ("NPP"). The NPP is available on request (see section 14). If this Privacy Policy and the NPP conflict regarding PHI, the NPP controls.
Information you give us. When you contact us, request services, refer someone for care, or apply for a job, we collect the information you provide, such as your name, telephone number, email address, mailing address, the nature of your inquiry, and, for service inquiries, information about the care needs of you or a family member. Job applicants also provide employment history, credentials, license and certification information, and the information needed to complete the caregiver background checks Wisconsin requires (Wis. Stat. § 50.065).
Information collected automatically. When you use the Site, we and our service providers may collect your IP address, browser and device type, pages viewed, referring website, dates and times of visits, and similar data through cookies and comparable technologies (see section 8).
Communications. We keep records of the calls, voicemails, emails, and text messages you exchange with us, including the date, time, and content of text messages and your text-messaging opt-in and opt-out status. Where permitted by law, calls may be monitored or recorded for quality and training.
Information from others. For clients, we may receive information from the person who refers you, your managed care organization or case manager, your physician, and your family members or legal representative. For applicants, we receive information from references and background-screening providers, with your authorization where required.

Service providers. We use companies that host the Site, provide our telephone and text-messaging platform, operate our care-management, scheduling, and electronic visit verification software, provide electronic signatures, onboarding tools, and background screening, and support our operations. These companies may use personal information only to perform services for us, are bound by written contracts (and, where PHI is involved, by HIPAA business associate agreements), and may not use it for their own purposes or marketing.
Payers and regulators. We share information with Wisconsin Medicaid (ForwardHealth), managed care organizations, and state agencies as required to deliver and be paid for services and to maintain our certification, subject to the NPP.
Legal requirements and safety. We may disclose information to comply with law, subpoenas, court orders, or governmental requests; to meet mandatory reporting obligations; to enforce our agreements; or to protect the rights, safety, or property of any person.
Business transfers. If SPPC is involved in a merger, acquisition, financing, reorganization, or sale of all or part of its assets, personal information may be transferred as part of that transaction, subject to this Privacy Policy and, for PHI, to HIPAA.
At your direction. We share information with others when you ask us to or consent.
Mobile numbers and text-message consent. No mobile opt-in or text message consent will be shared with third parties or affiliates. We do not sell, rent, share, or otherwise disclose your mobile telephone number or your SMS consent to any third party or affiliate for their marketing or promotional purposes. Information related to your text-message opt-in is shared only with the service providers that deliver text messages on our behalf (for example, our telephone and messaging platform provider), solely so they can deliver those messages. We do not purchase or use third-party lists to send text messages.
Program. When you opt in, you agree to receive conversational and informational text messages from Support Plus Personal Care LLC at the mobile number you provide. Message topics include replies to your inquiries; visit scheduling, confirmations, and changes; care-coordination logistics; onboarding, document, and signature requests; and, for employees and applicants, shift, schedule, hiring, and employment-related notices.
How you opt in. You opt in by checking the SMS consent box on one of our forms, by signing a consent in your intake or onboarding paperwork, by giving verbal consent that we record, or by texting a start keyword to our number. Consent is not a condition of receiving services from SPPC or of employment.
Message frequency varies. The number of messages depends on your services, schedule, and interactions with us.
Message and data rates may apply. Your mobile carrier's standard charges apply to messages you send and receive.
How to opt out. Reply STOP to any message to cancel. We will send one final message confirming your opt-out. You may also opt out by replying with any reasonable request to stop (for example, END, CANCEL, UNSUBSCRIBE, QUIT, or REVOKE), by calling 262-420-4008, or by emailing [email protected]. We honor opt-out requests promptly and in every case within 10 business days, and we treat an opt-out as revoking consent to all text messages from us. If you opt out, we will still contact you by other means when necessary for your care or employment.

Carriers are not liable for delayed or undelivered messages.
Eligibility. You must be at least 18 years old and the account holder or an authorized user of the mobile number. For clients who are minors or who have a legal representative, we communicate by text with the parent, guardian, or authorized representative who opted in.
Emergencies. Text messaging is not monitored around the clock and delivery is not guaranteed. Do not use text messages to report an emergency or an urgent care need. In an emergency, call 911.
Security of text messages. Standard text messages are not encrypted end to end and can be read by anyone with access to your phone or your carrier records. To limit risk, our messages contain only the minimum information needed (for example, dates, times, and requests to call us) and do not include detailed health information. By opting in, you accept these risks. You may request that we communicate with you by a different method at any time.
Changes. We may change the program, the number it uses, or these terms. Material changes will be posted on the Site or sent to you by text. Full program terms: supportpluspc.com/sms-terms.
The Site and our text-messaging program are not directed to children under 13, and we do not knowingly collect personal information from children under 13 online. Where SPPC serves a client who is a minor, we communicate with and obtain consent from the child's parent or legal guardian. If you believe a child under 13 has provided us personal information through the Site, contact us and we will delete it.
We will not deny services, charge different rates, or treat you differently because you exercised these rights.
We use cookies and similar technologies to make the Site work and to understand how it is used. We use Google Analytics, loaded through Google Tag Manager, to measure visits and clicks on our phone number, with Google signals and ad personalization turned off. We also use Google Ads conversion measurement and Curve, a marketing measurement service, to learn which of our advertisements lead to calls and inquiries. Most browsers let you refuse or delete cookies; doing so may limit some Site features. The Site does not respond to browser "Do Not Track" signals.
We maintain administrative, technical, and physical safeguards designed to protect personal information, including access controls, multi-factor authentication, encryption of data at rest and in transit within our systems, workforce training, and written policies. Electronic PHI is protected in accordance with the HIPAA Security Rule (45 CFR Part 164, Subpart C). No method of transmission or storage is completely secure, and email and standard text messages are not encrypted end to end.
We keep personal information for as long as needed for the purposes described in this Privacy Policy and for the periods required by law, including HIPAA documentation requirements (six years, 45 CFR § 164.530(j)) and Wisconsin Medicaid provider record-retention rules. We retain text-message consent and opt-out records for at least four years after the last message.
If a security incident affects your personal information, we will notify you as required by Wis. Stat. § 134.98 and, for PHI, by the HIPAA Breach Notification Rule (45 CFR §§ 164.400–164.414).
The Site may link to websites we do not control, such as payer portals or state agencies. Their privacy practices are governed by their own policies.
We may update this Privacy Policy. The effective date at the top shows when it was last changed. Material changes will be posted on the Site before they take effect.
