Getting Started

?
Question 1

Who Needs Home Care?

Home care can support adults who need assistance with daily living activities such as bathing, medication reminders, walking, eating, dressing, and mobility.

?
Question 2

When Is Home Care Appropriate?

Home care may be appropriate when physical or cognitive changes make daily tasks difficult or unsafe to manage alone. Service requirements depend on the care plan and payer authorization.

?
Question 3

Can I Call In My Own Referral?

We're happy to answer questions about our services. A physician's order may be required before care begins, depending on the service and payer requirements. Our assessment is a service-intake conversation, not a government eligibility determination.

?
Question 4

How Quickly Can Care Begin?

For most families, it typically takes about 2 to 3 weeks from the initial call to the start of services. This can vary depending on your situation, required approvals, and coordination with your physician's office. Contact us to discuss your needs and we'll do our best to accommodate your timeline.

?
Question 5

Can I Choose My Caregiver?

Oftentimes, yes. Many clients are able to choose a loved one, relative, or friend as their caregiver, with limited exceptions. We also work closely with families to match each client with a caregiver based on their needs, preferences, schedule, and personality. Your loved one's comfort matters to us, and if a match is not the right fit, we'll work to find a better one.

Our Services

?
Question 1

What Type Of Care Do You Provide?

We offer four core service programs — each tailored to different needs. Personal Care Services cover hands-on daily assistance. Supportive Care Services cover practical household and lifestyle support. VA Care is coordinated specifically for veterans. Home Care Services brings these options together in a comprehensive program.

?
Question 2

What Type of Care Do You Provide?

We provide non-medical home care designed to help clients stay safe, comfortable, and independent at home. Our services include personal care, supportive care, veterans care, and home care services.

?
Question 3

What Makes Support Plus Personal Care Different From Other Agencies?

We're a minority female-owned, Medicaid-certified agency committed to culturally sensitive, personalized care. We employ individuals with disabilities, conduct daily monitoring and reporting, and match every client with a caregiver based on their specific medical, cultural, and personal needs. Care isn't a transaction for us, it's a commitment.

Costs & Insurance

?
Question 1

How Much Does Home Care Cost?

Support Plus accepts Medicaid for eligible clients when services are authorized. Coverage, approved services, hours, and out-of-pocket costs vary by individual circumstances and program requirements.

?
Question 2

What Are My Payment Options?

We accept the following:

  • Medicaid (Title 19)
  • HMOs
  • MCOs
  • VA Benefits
  • Long-Term Care Insurance

Call us at 262-420-4008 to confirm whether Support Plus participates with your identified payer. The payer determines coverage and authorization.

Support Plus Personal Care is an independent Medicaid-certified home-care provider, not a government agency. The applicable government program determines eligibility, covered services, and service authorization. Support Plus does not determine government-benefit eligibility or submit benefit applications on behalf of the public.

?
Question 3

What If I Don't Have The Type Of Insurance You Accept?

Give us a call at 262-420-4008 and we'll help you work through your options. We want to make sure cost is never a barrier to getting the care your loved one needs.

Our Caregivers

?
Question 1

How Are Your Personal Care Workers Screened?

Every PCW completes a rigorous hiring process before being placed with any client, including a full application, thorough background check, reference check, comprehensive training, and in-person interview. Your loved one's safety is non-negotiable.

?
Question 2

What Training Do Your Caregivers Receive?

All caregivers receive thorough, condition-specific training before their first placement. We also conduct annual training to keep our staff current on emerging senior care issues, new care techniques, and best practices so the quality of care only improves over time.

?
Question 3

What If The Caregiver Isn't The Right Fit?

We take caregiver matching seriously but if a placement isn't working for any reason, we'll reassign promptly. Your loved one's comfort and trust in their caregiver is essential to quality care. Just call us and we'll make it right.
